Rental Agreement (Please Agree)

I acknowledge the rental fee covers normal maintenance.  I am responsible for the expense of repairs due to abuse/neglect of the instrument.  When an instrument is in need of repair or adjustment, please contact the instrumental teacher as soon as possible.  Repairs will be made in a timely manner and instruments will be returned to the student at school.  Students renting school instruments are required to participate in their school band or orchestra.
