I have read and fully understand the terms set forth in herein and related to participation in the Northbridge Youth Softball Association, Inc.
as well as that presented in the indemnification, medical authorization consent, code of conduct and photograph release policy. I agree freely and voluntarily without any inducement.  

Registration fees are used to offset the cost of league fees, team insurance and town fees. No refunds of registration fee payments will be issued for any reason, once player uniforms have been ordered, without a full review/decision by the NYSA B.O.D.
