Lost, Misplaced or Replacement Permits: $20.00 each
Electronic Disposal (TVS, Computers, Monitors, Microwaves): $30.00 per item/Limit 2 per day
Large Item Disposal (appliances, mattresses, furniture): $50.00 per item.
Bulky Rigid Plastics (toy cars, playhouses): $30 per item
Air Conditioners & Refrigerators: $50 per item

This application is for a permit that is valid through June 30, 2027. Current permits will expire on June 30, 2026.

Please return all of the following:
• A completed and signed application. Click Here for the Application. 
• A copy of the current registration for each vehicle for which a permit is being purchased must be provided along with the application.
• If your Millis address does not appear on your registration, you MUST provide a copy of your insurance indicating "principally garaged" in Millis

Permits are non-transferable and cannot be placed on any vehicle except the one for which it was purchased. Permits must be permanently affixed to the lower driver’s side front windshield. Permits are non-refundable.
