 Death Certificates 

Please call the Town Clerk's Office at 508-248-2249 prior to placing an order for any vital certificate, to make sure we have the certificate you are requesting.  

 

The Charlton Town Clerk's Office issues certified copies of the records of death of any person whose death occurred within the Town and for any Charlton resident who passed away in any other Massachusetts community and is still a resident of Charlton. If you are not certain whether a death certificate is filed with the Charlton Town Clerk, call the office for confirmation at 508-248-2249 before ordering.

The Registry of Vital Records and Statistics (RVRS) is providing interim guidance on the handling of Social Security numbers (SSNs) on death records in preparation for the implementation of Section 1 of Chapter 68 of the Acts of 2025, An Act Relative to Vital Statistics.


This law amends M.G.L. c. 46, § 1 by adding the following requirement: "For purposes of this section, the social security number in a record of death shall not be made available except upon request from a person with a legitimate need, as defined in regulations promulgated pursuant to section 4 of chapter 17." This provision takes effect on July 1, 2026.


Once the law is in effect, the full SSN on a death record may only be released to individuals who demonstrate a legitimate need. Please review the table below for the required documentation and upload the appropriate materials to our system.

Spouse/Domestic Partner Copy of your marriage certificate or registered domestic partnership listing the decedent
Parent A copy of your child's birth certificate listing you as the parent of the decedent
Legal Guardian An original certified copy of your court appointment
Child A copy of your birth certificate listing the decedent as your parent
Grandchild A copy of your birth certificate and your parent's birth certificate that lists the decedent as their parent
Grandparent A copy of your child's birth certificate that lists them as the parent and the deceased grandchild's birth certificate listing their child as the parent of the decedent
Sibling  A copy of your birth certificate that lists at least one common parent with the decedent and parents listed on the death record
Informant Must be listed on the death record as the informant
Funeral Director Licensed in accordance with M.G.L c.112 & 83 who is listed on the death record
Legal Conservator An original certified copy of your court appointment representing the decedent's estate
Legal Representative An original or notarized copy of your employment representing the decedent's estate by the client and your bar card
Government Official Agency photo ID; to fulfill an official government function
Person Identified through a Court Order An original certified copy of your court document stating access to the record with full SSN
Any other person with a direct tangible interest in the estate of the decedent At the discretion of the Registrar of Vital Records and Statistics or the local clerk where the death occurred
