Legal Waivers (parent must accept to participate)
As the parent and/or legal guardian of registered participant, a minor, I herewith authorize treatment by a qualified and licensed medical doctor in the event of a medical emergency which in the opinion of the attending physician, may endanger his or her life, cause physical impairment, disfigurement or undue discomfort if delayed. This authority is granted only after a reasonable effort has been made to contact me.
On behalf of myself and/or my minor child, hereby release and hold harmless the Town of Wrentham, Wrentham Recreation, Wrentham Elementary Schools, King Philip Regional Schools, all associated officers, employees, contact employees, and agents from any claims, causes of action or liability arising or relating in any way to any injuries that I or my child might sustain from my or my child’s participation in the listed activities including such claims or causes of action that I may now of have thereafter acquire (either independently or as a parent of said child) or that my child or hereafter may acquire.
